If you have referred a patient to our office and they have started treatment with us, let us know! We want to thank you for referring patients to our office. That is the highest compliment! We appreciate anyone you send to our office and we are glad that you have enjoyed your treatment so much that you have recommended us to one of your friends or family members.

Rules and Guidelines:

• You do not qualify for the program until the patient actually begins treatment with us.
• When the patient is filling out their new patient information forms they must put you down as their reference or the patient must state that they were referred by you during their initial phone call with us.
• You must either be a current or former patient or the responsible party of a current or former patient to qualify for the referral program.
• To participate in the program you must have a contract with us that is paid current.
• Please call our office if you think you qualify for our referral program.
• All above rules and guidelines are valid starting December 2, 2008.
• Referral Program rules and guidelines are subject to change at anytime.
